Since 2006, Shape Up San Francisco has been a driving force to prevent chronic disease through programs, policies, systems, and environmental changes in populations that experience the greatest health disparities. Collectively, we've made tremendous impact and have a great distance yet to go.

Hear from health warriors on the ground, policymakers and others as they discuss bold ideas for the future of chronic disease prevention and healthy communities.

Shape Up SF is staffed by the Community Health Equity & Promotion Branch of the SF Department of Public Health and since 2006, has been convening this multidisciplinary collaborative to reduce chronic disease health disparities that disproportionately affect African American, Latino and Asian populations in San Francisco. Shape Up SF’s mission is to advance health equity in San Francisco by collaborating with community on systems changes that increase nutrition security and active living.
